Youth and Family Ministry Fee information is as follows:

  • Traditional Program:    $50 one child  | $85 two children  |  $110 family maximum (Saturday PK-5th) 
  • Life Teen or EDGE:        $50 one child | $85 two children | $110 family maximum
  • Family Program:           $60 per family   
  • Summer Program         Registration will be available for the 2022-2023 year
  • Home Study:                 $60 per family*
  • Confirmation                 $60 per candidate
  • *Can be included in the family maximum of $110.

Re-registration info and re-registration form for students and families who were in any of the Faith Formation and Youth Ministry programs at Queen of Heaven parish during the 2020--2021 year.   Students who completed 10th grade in a Catholic High School or Faith Formation program can register for the Confirmation process with these forms as well. We will send additional program details as soon as they are available.     

 RETURNING FAMILIES: If you need to update your emergency information form that is required for Saturday, Edge and Life Teen, please do so at the Emergency Contact form link at the end of the new family/student information.                                               

New Family/student letter and Registration form*******NEW students being registered for in-person sessions that parents do not attend with them including kindergarten, first grade, transfers from a Catholic school, new to the parish, those from other parishes wishing to attend Faith Formation sessions here need to fill out the Emergency Contact form here.*   

THE SUMMER PROGRAM IS NOT AVAILABLE UNTIL 2022-2023 REGISTRATION BEGINS.                                                                                                                             The parishes of Queen of Heaven and 14 Holy Helpers hold a Summer Faith Formation Program intensive for students going into Grades 6 through 10. In keeping with the Diocesan curriculum, there is a family component as well.  The program is usually held in June, right after the students get out of school. The hours are usually 9 a.m. – 3 p.m., the first four days, and 9 a.m. – 1:30 on the last day.  Available parents are invited to attend various events during this program, especially Mass & lunch on the last day, AND parents and students are required to attend family sessions that occur a couple of times during the academic year.  For more information on the Summer Program please email [email protected] Summer Program Registration Form
